We provide links to guide patients and families to trusted, appropriate resources for advance directives, power of attorney, and health care proxy planning—helping ensure essential protections are in place before a crisis escalates. This isn’t just paperwork. It’s control, dignity, and peace of mind at the most vulnerable time in a person’s life.

Advance Directive
